All information must be completed to receive credit.
We will not be accepting warranty claims after the date of Friday, December 15, 2017, so that we may process and complete all warranty claims already received. If you have warranty claims that have been done after December 15, 2017, please hold on to them and submit parts and completed paperwork after the new year. You may fax your completed form to (808) 841-7222 or email to our Warranty Department.You may walk-in and drop-off your warranty claim form or email your completed form to firstname.lastname@example.org.
